Bristol NHS Foundation Trust - merger of North Bristol NHS Trust and University Hospitals Bristol and Weston NHS Foundation Trust
Our team was engaged three months ahead of the merger, because nobody yet had a clear answer to a basic question: which systems actually used ODS (Organisation Data Service) codes, how those codes got there in the first place, and how they fed statutory and operational reporting. Getting that answered before the two Trusts came together, rather than after, was the point.
That meant going system by system - 163 in total - to establish whether ODS was configured directly, arriving through integration, or both, then following it out to 15 external NHS Spine services including SDS, PDS and e-RS. Much of this depended on people rather than documentation: where a system owner didn't hold the detail, we tracked down whoever did, or dug it out independently through suppliers and interface specs.
The output was a full system inventory and data flow map that hadn't existed before, plus a mapping of 161 external reports from old ODS codes to new. Off the back of it, 195 new ODS codes are now reconciled and in active use, with the Trust reporting accurately to national bodies such as NHS England and HCAI.
